All providers on every tier

Provider access is no longer tier-gated. The response schema changed, and the Free rate limit is now lower.

Provider gating removed - every provider (javdb, missav, and future ones) is now available on every tier. Free, Basic, and Pro users get the same provider access.
Response schema changed - the response format now uses one shape across providers and tiers.
Free rate limit reduced - the RapidAPI Free tier now allows 100 requests per day instead of 500. This keeps the public tier usable without overloading the service.
